A. Plans for
total nursing care of critically ill patient
1.
Gives
skilled intensive nursing care to patients according to their individual needs:
utilizing the nursing process or scientific problem-solving approach.
2.
Alertness
in rendering cardiopulmonary resuscitation especially in emergency situation
like responding to emergency code Dr. CATS.
3.
Makes
a continuous assessment of the patient condition and make accurate recording of
unusual findings through the use of:
a) Monitoring
devices- interpreting ECG tracing and dangerous arrythmias and report to
resident physician on duty immediately for treatment or necessary action.
b) Hemodynamic
devices- monitoring Central Venous Pressure (CVP) and/or arterial line for
clients with congestion or impending shock; able to set appropriate equipments
and materials needed for CVP and arterial line insertion.
c) Direct
Observation- keen observant on the neurologic and cardiovascular changes of
patient and report immediately to resident physicians on duty for necessary
action.
d) Physical
examinations- identifies relevant signs and symptoms that will contribute
in establishing a diagnosis.
e) Knowledge
of the different laboratory procedures and its normal values and report to the
resident physicians abnormalities noted.
f) Adequately
informed about emergency drugs and interactions.
g) Knowledge
on modes of mechanical ventilation (CMV, SIMV and CPAP) ventilator
controls and settings (Tidal volume, Back-up rate, FI02, Peak Flow and
PEEP) and alarms (High-pressure alarms, Low-pressure alarms, apnea, low
O2 supply etc.)

Skillful
and able to perform the following:
a) Skillful
in inserting IV infusion and blood extraction when need arises
b) Use
of equipments like ECG machine, IV pumps, cardiac monitor, pulse oximeter,
defibrillators and respiratory assistance.
c) Skillful
in inserting Nasogastric Tube (NGT) and foley catheter aseptically.
d)
Assist physicians on inserting cutdown for CVP monitoring, subclavian
and intra-jugular catheter insertion and knows appropriate materials
needed especially types of lumen (double or triple lumen) as to the purpose
such for intravascular access for fluids, blood extraction and/or hemodialysis.
e) Able
to take 12-Lead ECG and shows how to identify fatal from non fatal arrhythmias.
f)
Knows to perform peritoneal dialysis and able to note (+) or (-)
cumulative difference in terms of intake or output.
i) Knows
how to set-up oxygen-compressed air in absence of nursing assistant especially
in emergency situation.
B. Have
good working relationship with other personnel in the unit.

to do task delegated by headnurse and substitute to headnurse when delegated.

physicians in diagnostic procedures: therapeutic and palliative measures.
3. Supervises
nursing assistant in the performance of their functions.
